APA 7 references for nursing care plan on Mac: cite sources in Word, 2026

For APA 7 references in a nursing care plan on Mac, cite each outside source in the text. Then list the full source at the end. Word's References tab does both steps. Zotero's Word plugin does both too, and its bibliography updates automatically. This page covers both tools, plus the checks that catch wrong details before your instructor does.

The short answer

  • Cite each outside fact in the text, then list the full source at the end.
  • In Word for Mac, go to References > Citations > Insert Citation > Add new source.
  • Zotero's Word plugin needs the Zotero desktop app open while you work.
  • Open every source and match it to its entry before you submit.
  • Keep patient details out of citations, and follow your employer's rules on any tool.

What citing sources in care plans costs you

Time lost to last-minute fixes

A citation fixed at the end takes longer than one fixed as you write. Rechecking a full list is slow, and a last-minute scramble adds stress to a task that should be simple.

Plagiarism questions

Missing or weak citations can be read as plagiarism, even when the copying was accidental. A messy reference list makes it harder for an instructor to see that you did the work.

A plan that is hard to check

A care plan cites evidence so the team can trust it and check it later. A plan with no sources, or with sources that do not open, makes that check harder for your preceptor and your team.

Small tasks on heavy shifts

Small admin tasks add up on a heavy shift. NCSBN reports that about 41.5% of nurses who intend to leave cite stress and burnout as the root cause. That figure covers nurses who intend to leave within five years, not all nurses.

Why it happens

  1. The rules and the tool are different skills

Students can know the citation rules and still not know where Word keeps the source dialog. Learning the style and learning the software are two separate jobs. Most guides cover one and skip the other.

  2. Care plans draw on many kinds of source

A care plan pulls from a textbook, a guideline, a journal article, and a hospital policy. Each type needs a different entry. In the Iranian study, students scored lowest on using NANDA resources, which points to weak work with reference material.

  3. Citations get added last

Most people write the plan first and add sources at the end. By then the wording has drifted from the source, and the link between them is lost. Adding the citation as you write prevents this.

  4. Charting takes a large share of the shift

Flowsheets took 31.11% of a 12-hour shift on average, based on February 2023 EHR vendor data. Cited prior work reports 631 to 875 unique data points per shift. A busy nurse has little time left for a reference list.

  5. Tools store details in different places

Word keeps sources in its own list, and Zotero keeps them in its library. A style chosen in one place may not match the other. Check the style setting in each tool before you build the list.

How to fix it

These work with or without any app.

  • Settle the style first

Ask your instructor or unit which style and heading they want, and get the answer in writing. Choose the style before you write. Changing styles late means rebuilding every citation and reference entry.

  • Capture each source as you read it

Save the title, authors, year, link, and date you opened the page while you read. A few minutes per source saves a long hunt at the end. Keep the list in the same folder as the PDFs.

  • Use the built-in citation tool

Use References > Citations > Insert Citation in Word, not typed brackets. A citation inserted with the tool stays tied to its stored source. Typed brackets drift out of step when you edit the text.

  • Check author, year, and title first

Open each source and match the author name, year, and title to your entry. Start with these three fields, because they are the easiest to mix up. Fix any mismatch before you check the rest.

  • Paraphrase with the source closed

Close the source, write the idea in your own words, then reopen the source to check the meaning. Add the citation in the same step. In the Swiss survey, 61.7% rated a light rewrite with a citation as acceptable, compared with 33.3% without one.

  • Read the Zotero troubleshooting page first

If you use Zotero, keep the desktop app open, install the Word plugin from Zotero's Cite settings, and restart Word. Zotero's troubleshooting page covers the last-resort steps. Read it before you start a long document.

  • Ask a classmate to find two sources

Have a classmate open two of your sources using only your reference entry. If they cannot find one, fix the entry before you submit. This costs nothing and catches typos that you will not see.

What a cited care plan needs to show

A care plan links each goal and intervention to the evidence behind it. An in-text citation shows where a claim comes from. The reference list at the end gives the full details for each source. Your reader should be able to find every source you name.

Your school may ask for APA 7, or your unit may have its own template. When the two disagree, follow your instructor or your unit's policy. This page does not list every APA rule. For exact punctuation, italics, and capitals, check the APA Style manual or your school's guide.

Here is what one line can look like. Intervention: reposition the patient on a set schedule (Author, Year). The reference list then holds the full entry for that author and year. Replace the placeholders with the details from your real source.

How common citation errors are for nursing students

Most of the public data here comes from nursing students, not working nurses. In a Swiss survey, 61% of students had been in doubt about how to act ethically on citation and plagiarism at least once in the past 12 months. Yet 78.5% said they understood the official standards. Knowing the rule and applying it under deadline pressure are different skills.

Only 79 of 343 invited students answered, a 23% response rate, at one university. Treat these numbers as a warning sign, not a national count. They still show that confusion is common enough to plan for.

Find your sources before you write the plan

Pull sources before you write, not after. Writing first and hunting later leads to vague claims with no source behind them. Keep one folder for each care plan. Save the PDF, the link, and a short note in the same place.

  1. Open each article in Chrome, in its own tab.
  2. Copy the title, authors, and year into a plain text list.
  3. Copy the DOI or the web address into the same list.
  4. Write one sentence on what the source says and where it fits in your plan.
  5. Note the date you opened any web page, because pages change.

Chrome helps with this step. Hold ⌥ ⌘ and ask Lazur about the page when a section is unclear. Lazur answers out loud from what is on your screen. It does not save the source or check the reference for you. You still do that part. See Lazur in Chrome.

Cite in Word for Mac with the References tab

Start with the built-in tools. Word for Mac can insert in-text citations and build a bibliography from the sources you enter. Microsoft's guide was written for Word 2019 for Mac. Newer versions may label things a little differently, so match the names on your screen.

  1. Click at the end of the phrase you want to cite.
  2. Go to References > Citations.
  3. Click Insert Citation, then choose Add new source.
  4. In the Create Source dialog, choose the source type that matches your reference.
  5. Fill in the fields the dialog asks for. Repeat for each source you cite.

Manage Sources sits in the Citations group. It opens the Source Manager, where you add, edit, or delete sources. If an author name or year is wrong, fix it there, not in the body text. Then check the entry against the original source.

To build the list, go to References, then click the arrow next to Bibliography. Choose Bibliography or Works Cited. Works Cited is the label many MLA guides use. APA calls the list References, so change the heading to match your guide after Word inserts it.

Use Zotero for longer care plans and papers

Zotero is a reference manager with a plugin for Word. It helps when you cite the same sources many times, or when you write a longer paper. You collect sources once in the Zotero desktop app, then insert them into Word. The plugin adds a Zotero tab to Word.

Add/Edit Citation inserts or edits a citation. Document Preferences sets the citation style, so choose APA there and confirm the edition in the style name. Add/Edit Bibliography inserts the list, and it updates automatically. Refresh picks up changes you made in Zotero.

Zotero must be open while you work. Its troubleshooting page says the desktop program must run. The Zotero website and the browser Zotero Connector do not count.

Fix the Zotero tab when it does not appear

Work through these steps in order. Each one comes from Zotero's troubleshooting and installation pages.

  1. Open Zotero's settings, go to the Cite tab, and find the Word Processors section.
  2. Click the install button for the Word plugin, then restart Word.
  3. In Word, open Tools > Templates and Add-ins. Zotero.dotm should appear under Global Templates and Add-ins.
  4. If the tab still does not show, delete Zotero.dotm from the Word Startup folder and restart Word.
  5. Confirm the tab is gone, reset the startup folder to its default, then reinstall the plugin.

Zotero's official troubleshooting pages do not cover Mac automation or macro permissions. If you hit those errors, search the Zotero forums before you change any system setting.

Check each reference before you submit

Check every entry against the original source. AI tools can produce a reference that looks right but points to a paper that does not exist, or has the wrong year or authors. Open each source. Compare the title, authors, year, and link. If you cannot find it, remove the citation or replace it with a source you can open.

Check the reverse direction too. Each in-text citation needs a matching entry, and each entry should appear in the text. Press ⌘F and search for each author name, one at a time. A source with no hit in the text may be an entry you can remove.

Patient data and employer rules

A care plan citation should point to published research, guidelines, or course material. It should never name a patient, a room, a date of birth, or a record number. Keep those details out of your sources folder too. Do not paste them into an AI tool to format citations.

Nurses work under employer rules on patient data. Follow those rules before you use any app, including Lazur. If your employer has not approved a tool for patient information, use it only for text with no patient details.

Mistakes that lose points in a care plan

  • Citing a general textbook for a specific claim, without checking for a more direct source.
  • Listing a source you never cite in the text.
  • Mixing styles, such as a Works Cited heading on an APA page.
  • Relying on memory for author names and years instead of opening the source.
  • Forgetting the date you opened a web page, so a reader cannot tell which version you read.

What the care plan research says about the skill

In an Iranian study of 248 nursing students, the mean care plan writing score was 3.35 out of 5, with a standard deviation of 0.57. The lowest areas included using NANDA resources, therapeutic communication, interdependent collaboration, and setting time frames for evaluation. GPA, age, semester, and daily study time predicted scores.

The model explained 61% of the variance. The study is cross-sectional, so it shows links, not cause. Its authors recommend practical care plan writing training and active teaching methods. Ask your instructor for a worked example, then copy its structure. For wording help, see the NANDA diagnosis guide.

Where Lazur helps, and where it does not

Lazur is a voice assistant for the Mac. Hold ⌥ ⌘ and ask about anything on your screen, and Lazur answers out loud. Ask where a button is, and Lazur draws a circle and a label around it. Ask how to do a task, and it guides you one click at a time, waiting for each click.

Lazur does not click buttons or operate Word for you. It cannot insert your citations, and it is not a plugin for Word or Zotero. Use it to find menus, explain a term, or draft text that you then check. Lazur's research runs in its own Chrome tabs and comes back with a summary and a report. You still open each source yourself. See Word tips for nurses.

Next steps for your next care plan

Start with one plan. Build the source list first, then insert each citation as you write. Check the reference list once at the end, and again after your instructor's comments. If your school uses Google Docs, see Lazur in Google Docs for voice help and dictation in your browser.

A plan you can start this week

  1. Day 1

    Set up the tools

Open Word and check that the References tab shows Citations and Bibliography. If you use Zotero, install the Word plugin from Zotero's Cite settings and restart Word. Then check Tools > Templates and Add-ins.

  2. Day 2

    Confirm the format

Ask your instructor or unit which style and heading they want, and write the answer down. If you use Zotero, set that style in Document Preferences before you insert anything.

  3. Day 3

    Capture sources

Collect a few sources for the plan. Record the title, authors, year, link, and date opened for each one. Save the PDFs in one folder, with the year in the file name.

  4. Day 4

    Cite as you write

Insert each citation the moment you add a fact. Use Insert Citation in Word, or Add/Edit Citation in Zotero. Do not leave gaps to fill in later, because gaps are where errors start.

  5. Day 5

    Check and submit

Open every source and match it to its entry. Remove any entry you cannot find. Insert the bibliography, rename the heading to match your guide, and save a copy before you use any Unlink command.

Questions people ask

Do I need APA 7 for every nursing care plan?

Not always. Your school, instructor, or employer sets the format. Ask for the required style in writing. If they name APA 7, use it through the whole document, including the reference list.

Where does the reference list go in Word for Mac?

At the end of the document, under a heading. Go to References, click the arrow next to Bibliography, and choose the option that fits. Then change the heading to match your guide.

Does Lazur insert citations for me?

No. Lazur explains, points, guides, and drafts text. It does not click buttons or operate Word for you. You insert and check each citation yourself. Lazur is not a plugin for Word or Zotero.

Can an AI tool write my references?

It can draft text, but it can also invent details. Open each source and compare the title, authors, year, and link. Do not paste patient details into any tool unless your employer allows it.

My Zotero tab still will not appear. What now?

Start with the install button in Zotero's Cite settings, then restart Word. Check Tools > Templates and Add-ins. If the tab is still missing, Zotero's troubleshooting page covers deleting Zotero.dotm from the Word Startup folder.

Does Lazur work on Windows?

No. Lazur runs on macOS, on Apple silicon and Intel Macs. Windows is planned, but it is not available yet.
